The second round of Spirit Wear subject to “tax free” order
status will be closed out on November 15 ;please disregard the October 17 deadline date reflected on the form. We
have order forms from both of our Spirit Wear business partners; Not Just Soccer (merchandise photos reflected
below) and Sew Much Fun (no photos available; see form for item descriptions). If you missed
the first opportunity to order, recently joined a team, or would like to make an additional purchase please use the attached
forms and Email your order to kacotter1@verizon.net. Checks should
be made payable to CHSHA; please contact Kim Cotter to coordinate delivery of payment. Delivery of orders can be expected
